Performance-based funding (ISF) is an activity-based funding scheme from the state to the regional health organisations. Codes in connection with digital home monitoring can be used, e.g. WMGA28 Digital, form-based patient follow-up and monitoring based on patient-reported outcome measures (PRO) or GDFC34 Digital home follow-up of remotely reported data for spirometry (registration and reporting must take place at least once per term, this applies even if the patient is not necessarily in direct contact with healthcare personnel at the hospital in the period).
